The care model allows physicians to see fewer patients while spending more time focusing on high-quality patient care and preventive health management. Description A healthcare organization in Mobile, Alabama is seeking a Primary Care Physician to provide comprehensive outpatient care to adult patients in a value-based care setting. The physician will evaluate, diagnose, and manage acute and chronic conditions while collaborating with a multidisciplinary care team to deliver patient-centered healthcare. This role focuses on preventive care, chronic disease management, and care coordination to improve patient outcomes.
