Job Description Job Description Summary: Individual Needs
Assessor (INA) completes the Louisiana Independent Living
Assessment (LILA) and /or NFCSP Assessment with senior citizens age
60 and over to determine eligibility for services provided by the
Jefferson Council on Aging, Inc. These services include Home
Delivered Meals, Material Aid, Homemaker Services, Personal Care,
and Respite Care. An initial assessment is completed in the home
upon receipt of referral and annually thereafter. Education: 2 year
degree or four years relevant experience Responsibilities: Call new
and existing program participants to conduct the Louisiana
Independent Living Assessments. Meet in the home with individuals
and /or families to view the home and obtain assessment signatures.
Enter participant information in JCOA’s database. Turn completed
assessments in to the main office every two weeks. Attend trainings
and meeting as needed. Supervisory Responsibilities : This job has
no supervisory responsibilities. Qualifications: Team mentality
Able to pass a criminal and driver's license background inspection
Must have an automobile in good running condition Knowledge of
operation of standard office equipment Maintain confidentiality of
all client information Ability build rapport with elderly
individuals Exceptional communication skills, written and verbal
Ability to work independently Ability to respect cultural, social
and racial differences Emotional maturity, and the ability to be
non-judgmental Attention to details and accuracy Flexible and
Adaptable Computer Skills: Must be proficient in Microsoft Office
Suite and have general computer knowledge Remote internet access is
highly recommended. Physical Demands: The physical demands
